About Root Cause Analysis

The Sologic RCA method draws on the most advanced cause-and-effect methods and best practices focused on steps that deliver value and save time.

The Sologic root cause analysis method is universal, scalable, practical and intuitive for RCA novices and experts alike. It consists of logical, complete and accurate processes and terminology that’s aligned with how the world actually works. It has been specifically designed to take the focus away from non-specific causes, opinion, blame and punishment by shifting the focus to systemic causes, objectivity, collaboration and data.

The Sologic method enables RCA facilitators and teams to effectively and efficiently identify true causes and effective solutions. This method enables teams to make informed business decisions and directly supports achievement of business goals.

The Sologic RCA method?

  1. Gather and manage data Identify sources of evidence and team members who can provide this evidence. Ensure there is provision for secure data and document storage for each RCA investigation

  2. Create the problem statement Identify and agree upon the focal point of the problem. You will then gather information to answer: when the problem happened, where the problem happened, and what the actual & potential impact was

  3. Analyse cause and effect In most -- if not all -- cases, a problem has more than one cause. By analysing the cause and effect you can identify: what caused the effect, if it always causes the same effect every time this problem occurs, if it a transitory cause (points of change) or if it is a non-transitory cause (things, their properties and their status). Also, you can look at Omissions, which are causes that should have been in place, but were not. Finally, you need will need to support causes with evidence and identify termination points

  4. Brainstorm solution Your team should brainstorm ways to control, alter and prevent causes. They should look for both corrective and preventative actions. Once these actions have been decided upon, your team should apply the solution criteria of: A) Effectiveness, B) Ease of implementation, C) Return on investment and D) Potential negative impacts. Finally, your team should assign action items, responsibilities and due dates

  5. Produce the final report The final reports should include an executive summary, problem statement, cause-and-effect chart, solutions, team info and key dates.
